Form for Player Reports

Hey there Guys. I noticed that this Forum doesn't have a form for Player Reports

So my suggestion is, to add a Form for it.

It could look like this:

Player Report:

Your ingame name: (Reap)

The reported player's in game name: (Example: Madvillayo)

Violated Rule: (Teaming, Hacking, Flaming, Admin abusing, etc)

Evidence: (Video or Pictures)
